Discretion begins with the first conversation.

Information collected
The membership inquiry requests basic contact details, professional or family-office context, general areas of interest, timing and referral information. The form is intentionally designed not to collect medical records, diagnoses, medications, test results, insurance information or other detailed health information.
How inquiry information is used
Information submitted through the site is used to review fit, respond to the inquiry, arrange an introductory conversation, maintain appropriate business records and protect the integrity and security of the site. Submission does not establish a physician-patient relationship, a treating relationship or membership.
Sensitive health information
Please do not submit medical records or detailed health information through the public inquiry form. If a membership relationship is accepted, any required records, releases or sensitive information will be requested through a separately designated process appropriate to the engagement.
Sharing and advisor participation
Inquiry information may be accessed by authorized personnel and service providers who support the site and business operations. It is not sold. Information is not shared with a wealth advisor, family office, attorney or other trusted professional merely because that person made an introduction; family participation and permissions are confirmed separately.
Retention and safeguards
Reasonable administrative and technical safeguards are used to protect inquiry information. No internet transmission or storage system can be guaranteed completely secure. Information is retained only as reasonably needed for the purposes described here, applicable obligations and legitimate business records.
Your choices
You may request correction or deletion of membership inquiry information, subject to legal and recordkeeping requirements, by contacting concierge@familyofficehealthadvisory.com and identifying the prior submission. Do not include medical details in that request.
Important service boundary
Family Office Health Advisory is not an emergency service, health insurer, legal advisor or replacement for treating clinicians. Call 911 or the appropriate local emergency service for an emergency.
