What does a healthcare navigator do?
FOHA connects the information surrounding care. The engagement can address a specific decision or provide continuing organization for a family with multiple physicians and priorities.
- Gather authorized records and identify missing information.
- Prepare a Consolidated Medical Report and care-team map.
- Create questions and focused briefs for selected appointments.
- Research treatment evidence or potential coverage benefits.
- Track referrals, results and agreed next steps.
When might a family need navigation?
Navigation may be useful when a new diagnosis brings several opinions, when care spans multiple health systems, or when an aging parent needs more organized support. A clearly defined question helps determine whether FOHA is a good fit.
- What decision needs to be made, and by when?
- Which records or recommendations are missing?
- Who owns the next action?
- Who has permission to participate?
How does FOHA work with your existing doctors?
We build summaries and questions around the clinicians the family already trusts. With authorization, we coordinate relevant information and clarify outstanding steps. FOHA's advisory membership does not provide primary care, prescribe medication or replace specialist advice.
| Responsibility | Treating clinical team | FOHA advisory |
|---|---|---|
| Diagnosis and treatment | Directs clinical care | Provides context and preparation |
| Medical records | Creates clinical records | Organizes authorized records |
| Important decisions | Recommends clinical options | Researches questions and supports understanding |
| Next steps | Orders and manages care | Tracks agreed actions and coordination |
What does the family receive?
Deliverables depend on the written engagement. They can include a health summary, an appointment brief, a research dossier and an action list showing the responsible person and next milestone. Record gaps and uncertainty remain visible rather than being presented as resolved.
Where can FOHA provide advisory support?
Advisory conversations can take place remotely, with the location, permissions and limits of each engagement confirmed in advance. The service does not imply a treating relationship across jurisdictions or guaranteed access to a particular physician.
What else should you know?
Is healthcare navigation the same as concierge medicine?
No. Concierge medicine generally centers on a treating medical practice. FOHA's advisory centers on records, preparation, research and coordination across the family's existing clinicians. The services can work alongside one another.
Can I keep my current doctors?
Yes. FOHA is designed to work around the treating physicians and specialists you already use. Any new referral remains a separate clinical relationship.
Do you prepare a Consolidated Medical Report?
Yes, when included in the engagement. It summarizes authorized records, relevant history, medications, care-team information and open questions, with references to the underlying documents.
Can you help prepare for a second opinion?
FOHA can organize records and questions for a second-opinion visit. The consulting clinician provides the medical opinion; access, timing and coverage must be confirmed separately.
Can you help with a parent's healthcare?
Potentially, with the parent's permission or appropriate legal authority. We confirm who can access information and coordinate decisions before accepting a family member's instructions.
Will you guarantee faster appointments or better outcomes?
No. We provide defined advisory and coordination work. Appointments, coverage, cost savings and medical outcomes cannot be guaranteed.
What should I send in a membership inquiry?
Provide contact details and broad areas of interest only. Do not send diagnoses, records, medication lists or insurance identifiers through the public inquiry form. Records are requested through a separately designated process.
How are membership scope and pricing determined?
The family discusses fit and needs with FOHA. The written agreement specifies the fee, included services, response standards and limits. Contact the Concierge team for current terms.
